The Perk Treatment by Hydrafacial is made available to you by HydraFacial LLC and its affiliates (collectively, “HydraFacial”) and Sephora Australia Pty Ltd and its affiliates (collectively “Sephora”).
By completing a booking for the Perk Treatment by Hydrafacial service, you acknowledge and confirm that:
1. As of the treatment date, you:
a. Are not pregnant or breastfeeding
b. Do not have any open lesions or cold sores
c. Do not have active acne or infections in the treatment area
d. Do not have an active sunburn on your face or around your lips
e. Do not have skin conditions such as eczema, dermatitis, or rashes
f. Do not have autoimmune diseases such as lupus
g. Do not have viral concerns such as HIV or hepatitis
h. Have not undergone anticoagulant therapy
i. Do not have melanoma or any lesions suspected of malignancy
j. Have not used Accutane in the past 6 months
k. Have not had aesthetic fillers, injectables, or laser treatments in the last 14 days;
2. Although the Treatment is designed to produce a certain result, individual client experiences and results may vary from person to person, and both HydraFacial and Sephora have made no particular representation or guarantee about the Perk Treatment by Hydrafacial and/or their results, safety or suitability;
3. You fully understand the nature, benefits, risks and potential side-effects of the Perk Treatment by Hydrafacial as described and/or explained to me as part of the booking workflow, under these Hydrafacial Additional T&Cs, or by the Sephora personnel who will be administering the Perk Treatment by Hydrafacial (“Sephora Skincare Experts”), and will immediately consult a physician if any side-effects persist;
4. It is your responsibility to follow the directions provided by the Sephora Skincare Experts during the Perk Treatment by Hydrafacial and any after-care advice, and you agree that HydraFacial and Sephora shall not be held responsible for my failure to do so and/or for any complications arising through no fault of HydraFacial and Sephora or factors beyond the reasonable control of HydraFacial and Sephora; and
5. The Perk Treatment by Hydrafacial is not a medical or clinical treatment, nor is it intended for the diagnosis, prevention, monitoring, treatment or alleviation of any diseases or injury, and the Sephora Skincare Experts’ directions and/or after-care advice do not constitute professional medical advice.
I acknowledge that neither Hydrafacial nor Sephora and/or each of their officers, employees and representatives shall not be liable for any losses or damages which I may suffer in connection with my voluntary election to receive the Perk Treatment by Hydrafacial, and I hereby waive all actions, claims, and demands arising out of my participation.
